Do you have an “Advance Directives”? An example of Advance Directives is a document that instructs healthcare workers what to do in case of an emergency. Like a DNR (Do Not Resuscitate) mostly drafted by an attorney. If you do, we request that you bring them, we will make a photocopy and attach them in your medical records, but keep in mind that our policy in the facility is to always apply emergency measures and call 911 in case of a life-threatening emergency.
-
If you have a list of current medications you are taking, please provide us with a copy, we may reconcile them if we need to, or bring the actual medications you are currently taking so we can make a reconciled list for you.
(No signature from the Patient, Insured, or Patient's Authorized Representative is required at this time. Official copies of the required forms will be provided for signature upon arrival at the surgical center.)
